2. Copy of 30 days - most recent paystubs
3. Proof of Other Income (SSI, Child Support, Etc.) if applicable.
4. If you are Self Employed, provide copies of the last 2 most recent tax returns.
5. Pet photo and records (shots, country license, etc) if applicable
6. If you have an Emotional Support Animal please request the application. In addition we will need a photo of the animal, county registration, reasonable accommodation letter
